In addition to the feedback you receive at the Mid-Point Gatherings, through your check-in calls and other discussions with participants, you will want to conduct a formal written evaluation at the completion of the program. Although you can implement a paper-based survey, one of the easiest ways to manage this is through an online survey tool like Survey Monkey or Zoomerang. Your chances of accurate information and a prompt return are enhanced by sending the survey out no later than one week following the graduation ceremony or other final event. You will want to get feedback regarding each aspect of the program as a whole. Evaluation questions for program areas that the IAA includes are:
Questions in each of these areas will provide you with both formative (process) and summative (outcome) evaluation data that will help you strengthen future programs. It can also help you make the case for related fundraising efforts. A template of the IAA's Final Evaluation is included here for your assistance. You can use these questions alone or in addition to your own. You will want to transfer them to a different document or online survey tool of your own design.
